2. Search for the name, then hit search.

3. Copy the URL.

4. On the bottom of the page click the “Remove my information” link.

5. Paste the url and type the email address you are using for your opt outs, complete the captcha, and click the "Send" button.

6. Success! You have submitted your removal request successfully.
