How to Opt-Out of HealthWiseData in 2024

1. Go to www.healthwisedata.com.

2. On the bottom of the page click the “Your privacy choices” link.

3. Fill in the form with your opt out preferences, and your personal information. When done, hit the "Submit for Review" button.

4. Success! You have submitted your removal request successfully.
